FAQs about Non-surgical Jaw Reduction by Btx

Will my skin sag after non-surgical jaw reduction by Btx?

No. The difference in jaw slimming is visible but not so great as to cause excess skin, such that it sags. As mentioned, it’s a natural looking effect, unlike surgical jaw reduction where people will most likely be able to tell.*

When I do non-surgical jaw reduction by Btx, will I become addicted to the procedure?

Only if you’re addicted to looking better than before. The procedure itself does not produce any form of dependence or tolerance to btx. If you are satisfied with the results or decide not to continue with it, you do not have to repeat the procedure any more than you so wish.*

Does the effect of non-surgical jaw reduction by Btx become permanent after a while?

This is a question that is still under major debate. Some believe that with prolonged weakening and atrophy of the masseter muscles, it becomes difficult to build it up again, so that the effect becomes more permanent. There may even be bone remodelling of the jaw due to changes in muscle action on the angles of the jaw. However, this effect is at best variable and uncertain. If you are keen to upkeep your beautiful V-shape face, it is still a good idea to maintain it with sufficient doses at regular intervals, depending on your doctor’s prescription.*

I did the non-surgical jaw reduction by Btx. Why do I not see the result?

If you do not see the results from your recent non-surgical jaw reduction by Btx, here’s a checklist to find out why:

1. Was it a reliable treatment? This may seem rather common sense, but it is important to know that your treating personnel is licensed and trained to perform this procedure, as well as use approved brands of botulinum toxin. At the date of this article, 3 brands of botulinum toxin: Btx, Dysprt and X’min are FDA (USA) and HSA (Singapore) approved. No unlicensed spa or beautician should be doing this procedure for you.*

2. Has it been 2 months since your non-surgical jaw reduction by Btx? Results are not instantaneous. It takes an average of 2-8 weeks to see good results.*

3. Was the dose of botulinum toxin enough? Low doses effect small changes, so that you are unable to tell the difference. Perhaps you need to check how much botulinum toxin was used and whether you need a top up.*

4. Is the reason for treatment correct? There are 4 reasons to not having a slim face: big bone, fat under the skin, buccal fat pad on the cheeks and masseter muscles at the angles of the jaws. Non-surgical jaw reduction by Btx only resolves the last one. You need other procedures to treat the other 3 causes. Occasionally, it is more than one reason. The doctor can tell you which is your real problem and recommend accordingly.*

5. Do you have antibodies to the botulinum toxin? This is very rare but possible. If you have antibodies to the botulinum toxin, the injected Btx will have been removed by the body and rendered ineffective at the point of injection. In that case, consider other forms of treatments. You may also consider trying Xeomin which is less antibody-inducing than the rest.*

Skin Tightening for Face Slimming

You may also consider slimming the face by skin tightening (such as Thermage, Hironic) or skin lifting by threadlift which corrects droopy skin. Excess droopy skin can make the face look boxier and more square than it actually is. A short or stubby chin is the nemesis of the ideal face shape for beauty. Using hyaluronic acid filler injections, you can reshape your chin, therefore improving both your side and frontal profile.*

Hyaluronic acid filler is a proteoglycan similar to what is also found in our skin, eyes and joints. It is highly reversible, either by dissolution or natural breakdown by the body over months, and has been found to be very safe for facial contouring.*

Why do Chin Filler at Tiffiny Yang Aesthetics & Surgery?

For chin contouring, it only takes one, at most two injections, with Dr Tiffiny Yang’s 1-point V shape face technique, therefore minimising the pain with almost no downtime.* For first-time fillers, an average of 1-2cc of filler is sufficient. Pain is minimised with the use of filler that contains lignocaine to numb the injected areas.*

Chin filler augmentation can create a more defined 3D shape of the chin– not only able to bring the chin forward for those with receding chins (retrognathia) but also make it sharper and/or longer for those with short, small chins (micrognathia). How is it done?

What are fillers?

How many types of fillers are there?

In a nutshell, fillers can be classified into temporary, semi-permanent and permanent fillers. Our favourite is the hyaluronic acid (HA), which is a temporary filler lasting between 9-12 months. It’s made of glycosaminoglycan polysaccharide and is not just found to be naturally occurring in the body as universal extracellular matrix– it’s also chemically identical across species and therefore, is very low on antigenicity, i.e. less likely to cause bad immune reactions. In other words, it’s very safe. There are many other brands in the market that are also similarly good in quality. Be sure to check with your doctor on this.*

There are also other type of fillers that are non-hyaluronic acid based, such as poly-L-lactic acid, calcium hydroxyapatite and poly-caprolactone. They essentially work by having an initial bulking effect plus an additional benefit of stimulating collagen synthesis wherever they are placed. This enhances their volume enhancing effect in the mid-term, so you may have to wait a little for the full effect. However, they may not necessarily last longer than current, newer type hyaluronic acid fillers. An alternative would be to choose a more viscous hyaluronic acid product, which your doctor can recommend to you.*

Semi-permanent fillers last the longest. Examples include polyacrylamide gel (i.e. plastic), and liquid silicone, which is the longest lasting of all fillers. These can last for years. However, the longer lasting the filler, the more likely it can cause complications such as lumps and scarring.*

Why did the facial filler seem to decrease so quickly in the first 2 days after injection?

Hyaluronic acid (HA) occurs naturally without cross linkages. In this form, it is easily broken down in the body in 1-2 days. The HA that remains are those that are still cross-linked, which prevents its breakdown, and this is the form that lasts. Therefore, your facial filler with HA can seem to decrease quickly in the first 2 days before it plateaus into a steady state. The rest decreases slowly. Therefore, the more cross-linked the HA, the longer it lasts.*

There is also a part of the filler procedure that causes a little swelling, such as the local anaesthetic and the inflammation of local trauma. This quickly subsides in a few days, leading to a reduction of up to 20-30% in the final result.*

Should I always choose the brand of HA that lasts the longest to save cost?

The longer HA lasts, the more viscous it must be and therefore, the harder its feel may be or its chance of leaving residual lumps. Harder HA may not be suitable for all areas of facial correction, even though they last longer, because they may not be suitable for all areas or feel all that natural in others.*

Why does my nose have a bluish tinge after filler injection with hyaluronic acid?

If it isn’t a bruise, it can be due to the Tyndall effect, which occurs after injection of hyaluronic acid is done too superficially so that it shows through the skin, making it take on a bluish tint that is not easily hidden by makeup. It doesn’t happen often, though, and can be avoided by placing the filler in the correct plane. Newer improved fillers seldom give the Tyndall effect these days.*

After a facial filler injection, can I go for massage or wash my face?

Temporary fillers are moldable in the first 4 hours or so and they settle in approximately 2 days. In the first 4 hours, you shouldn’t apply firm pressure on the area of the filler or massage too hard. Gentle washing of the face is permitted. For the first 2 days, it is advisable not to apply prolonged pressure on the area of the facial filler e.g. sleeping on that side of the face– there is a slight chance it may shift. Thereafter, once it has set, you can do whatever you want and it should not change in shape.*

Myth: Fillers are harmful for the body since they are absorbed by the body.

Truth: Absolutely not. Hyaluronic acid, which is used as temporary filler, is a natural substance found in the skin, joints and eyes of the body. It is gradually broken down by the body and not harmful to health. In fact, some brands of fillers has obtained FDA’s approval for clinical use.*

Myth: Surgery is required to place fillers into the body.

Truth: Not at all. Facial fillers for the nose, nasolabial folds, cheeks, chin, etc. are easily placed into the respective parts via a few simple injections. There is no need to undergo the knife– hence the wonder of minimally invasive procedures. Even breast fillers only require 1 or 2 injection sites under the breasts. Minimal pain and minimal scarring, in just under 15min!*

Myth: You need to keep coming back for frequent top-ups once you do fillers.

Truth: Top-ups are required only because temporary fillers are naturally broken down by the body and absorbed, so its effect diminishes over time. They are not in the least bit addictive and you can choose not to top up again if you’d like. The filler will simply disappear and the part of the body will return to its original state as before filler was done.*

There can be incidences where the fillers last longer than expected in some people, especially after 1 or 2 top ups. This eliminates the need for frequent top ups over time.*

Myth: When the filler disappears, you’ll look worse than before.

Truth: Fillers do not change the original state of the body part it is placed into. When it is absorbed, that part of the body simply returns to how it was supposed to be. Of course, temporary fillers last up to a year or more and in that time, the body is still undergoing its own aging process.*

Truth: Amazingly, it is rather difficult to look fake with facial fillers. They can give dramatic natural results and yet escape others’ eyes unnoticed. Too many have told me how they wished people had noticed– even though in their own eyes, it was a great difference!*

Myth: Fillers are very painful.

Truth: Some pain is to be expected but it is usually minimal and quickly forgotten after the procedure. The pain is usually only of the injections and subsequently, a vague dull ache during filler injection. Are you having tear troughs, sunken eyes, dark undereye circles or eyebags?

Tear troughs are caused by sunkenness under the eyes, creating lines that may look like eye bags or dark circles. This is easily reversed with a non-surgical procedure – a simple injection with a very soft filler such as Jvderm Ultra and Rstylene Vital Light under the eye areas to restore and rejuvenate.* Eyes can also look sunken on the upper eyelids, making a person look haggard and older.

If you happen to suffer from dark eye circles at the same time, which commonly happens to people who have sunken undereyes, the treatment also improves the appearance and reduce the extent of dark eye circles. What are fillers?

How many types of fillers are there?

In a nutshell, fillers can be classified into temporary, semi-permanent and permanent fillers. Our favourite is the hyaluronic acid (HA), which is a temporary filler lasting between 9-12 months. It’s made of glycosaminoglycan polysaccharide and is not just found to be naturally occurring in the body as universal extracellular matrix– it’s also chemically identical across species and therefore, is very low on antigenicity, i.e. less likely to cause bad immune reactions. In other words, it’s very safe. There are many other brands in the market that are also similarly good in quality. Be sure to check with your doctor on this.*

There are also other type of fillers that are non-hyaluronic acid based, such as poly-L-lactic acid, calcium hydroxyapatite and poly-caprolactone. They essentially work by having an initial bulking effect plus an additional benefit of stimulating collagen synthesis wherever they are placed. This enhances their volume enhancing effect in the mid-term, so you may have to wait a little for the full effect. However, they may not necessarily last longer than current, newer type hyaluronic acid fillers. An alternative would be to choose a more viscous hyaluronic acid product, which your doctor can recommend to you.*

Semi-permanent fillers last the longest. Examples include polyacrylamide gel (i.e. plastic), and liquid silicone, which is the longest lasting of all fillers. These can last for years. However, the longer lasting the filler, the more likely it can cause complications such as lumps and scarring.*

Why did the facial filler seem to decrease so quickly in the first 2 days after injection?

Hyaluronic acid (HA) occurs naturally without cross linkages. In this form, it is easily broken down in the body in 1-2 days. The HA that remains are those that are still cross-linked, which prevents its breakdown, and this is the form that lasts. Therefore, your facial filler with HA can seem to decrease quickly in the first 2 days before it plateaus into a steady state. The rest decreases slowly. Therefore, the more cross-linked the HA, the longer it lasts.*

There is also a part of the filler procedure that causes a little swelling, such as the local anaesthetic and the inflammation of local trauma. This quickly subsides in a few days, leading to a reduction of up to 20-30% in the final result.*

Should I always choose the brand of HA that lasts the longest to save cost?

The longer HA lasts, the more viscous it must be and therefore, the harder its feel may be or its chance of leaving residual lumps. Harder HA may not be suitable for all areas of facial correction, even though they last longer, because they may not be suitable for all areas or feel all that natural in others.*

Why does my nose have a bluish tinge after filler injection with hyaluronic acid?

If it isn’t a bruise, it can be due to the Tyndall effect, which occurs after injection of hyaluronic acid is done too superficially so that it shows through the skin, making it take on a bluish tint that is not easily hidden by makeup. It doesn’t happen often, though, and can be avoided by placing the filler in the correct plane. Newer improved fillers seldom give the Tyndall effect these days.*

After a facial filler injection, can I go for massage or wash my face?

Temporary fillers are moldable in the first 4 hours or so and they settle in approximately 2 days. In the first 4 hours, you shouldn’t apply firm pressure on the area of the filler or massage too hard. Gentle washing of the face is permitted. For the first 2 days, it is advisable not to apply prolonged pressure on the area of the facial filler e.g. sleeping on that side of the face– there is a slight chance it may shift. Thereafter, once it has set, you can do whatever you want and it should not change in shape.*

Nasolabial Folds Filler Correction

Laugh lines are nasolabial folds that run from both sides of your nose to the corners of your mouth. They come with aging, when facial fat pads are lost and cause the skin to sag downwards and form prominent folds.

Correction by Filler

These are easily reversed with hyaluronic acid filler injections into the lines, volumising the area and minimising the appearance of such lines.* Hyaluronic acid filler is a proteoglycan similar to what is also found in our skin, eyes and joints. It is highly reversible, either by dissolution or natural breakdown by the body over months, and has been found to be very safe for facial contouring.

This procedure takes up to 30 minutes, with 2-3 injections per side from the corners of the nose to a little way down the nasolabial lines. Some massage over the area after the procedure minimises side effects.* There is no downtime.*

What are fillers?

How many types of fillers are there?

In a nutshell, fillers can be classified into temporary, semi-permanent and permanent fillers. Our favourite is the hyaluronic acid (HA), which is a temporary filler lasting between 9-12 months. It’s made of glycosaminoglycan polysaccharide and is not just found to be naturally occurring in the body as universal extracellular matrix– it’s also chemically identical across species and therefore, is very low on antigenicity, i.e. less likely to cause bad immune reactions. In other words, it’s very safe. There are many other brands in the market that are also similarly good in quality. Be sure to check with your doctor on this.*

There are also other type of fillers that are non-hyaluronic acid based, such as poly-L-lactic acid, calcium hydroxyapatite and poly-caprolactone. They essentially work by having an initial bulking effect plus an additional benefit of stimulating collagen synthesis wherever they are placed. This enhances their volume enhancing effect in the mid-term, so you may have to wait a little for the full effect. However, they may not necessarily last longer than current, newer type hyaluronic acid fillers. An alternative would be to choose a more viscous hyaluronic acid product, which your doctor can recommend to you.*

Semi-permanent fillers last the longest. Examples include polyacrylamide gel (i.e. plastic), and liquid silicone, which is the longest lasting of all fillers. These can last for years. However, the longer lasting the filler, the more likely it can cause complications such as lumps and scarring.*

Why did the facial filler seem to decrease so quickly in the first 2 days after injection?

Hyaluronic acid (HA) occurs naturally without cross linkages. In this form, it is easily broken down in the body in 1-2 days. The HA that remains are those that are still cross-linked, which prevents its breakdown, and this is the form that lasts. Therefore, your facial filler with HA can seem to decrease quickly in the first 2 days before it plateaus into a steady state. The rest decreases slowly. Therefore, the more cross-linked the HA, the longer it lasts.*

There is also a part of the filler procedure that causes a little swelling, such as the local anaesthetic and the inflammation of local trauma. This quickly subsides in a few days, leading to a reduction of up to 20-30% in the final result.*

Should I always choose the brand of HA that lasts the longest to save cost?

The longer HA lasts, the more viscous it must be and therefore, the harder its feel may be or its chance of leaving residual lumps. Harder HA may not be suitable for all areas of facial correction, even though they last longer, because they may not be suitable for all areas or feel all that natural in others.

Why does my nose have a bluish tinge after filler injection with hyaluronic acid?

If it isn’t a bruise, it may be due to the Tyndall effect, which occurs after injection of hyaluronic acid is done too superficially so that it shows through the skin, making it take on a bluish tint that is not easily hidden by makeup. It doesn’t happen often, though, and can be avoided by placing the filler in the correct plane. Newer improved fillers seldom give the Tyndall effect these days.*

Injection for Dynamic Wrinkle Lines

Having forehead lines, frown lines or crow’s feet? Do you hate your bunny lines or lipstick lines? Even pebbly chins can be solved.*

Forehead lines are caused by the action of the frontalis muscle, frown lines by the proteus and corrugator muscles and crow’s feet by the orbcularis oculi muscle. These dynamic wrinkles are caused by frequent skin creasing from the contraction and relaxation of active muscles.

In other words, wrinkles are caused by the movement of facial muscles over a period of time. The facial expression on our face causes pleating, creasing and folding of our facial skin. After years of similar facial movement in the same areas, the commonly moved facial line become wrinkles. Think of your facial skin as a piece of cloth, if the cloth is ironed over and over again along the same line, a permanent line or pleat will form. This is a wrinkle.

Wrinkles are usually found in between the eyebrows (also known as frown lines), forehead lines, lines along the cheeks (known as laugh lines), and near the corner of the eye (known as crow’s feet).

Wrinkles can be done away (at least for a time) easily*, with just a few injections into these active muscles. It acts by relaxing these facial muscles which are overworking to produce repeated skin creasing; once they are relaxed, they do not cause wrinkling of the skin anymore until the effect of btx wears off.* Each treatment can last up to 4-6 months before a top up injection is required.*

Improvement from wrinkles can be seen within approximately 5 days.*

It is a Myth that wrinkles, if left alone will go away. It will not, and will only get worse as the facial muscles which are creasing and folding that area of the facial skin will still be creasing and folding the skin, making the wrinkles worse. The Reality is that for wrinkles to go away, that facial muscle which is causing the wrinkle must stop folding and/or creasing that part of the facial skin. This treatment will temporarily stop the muscle from repeatedly creasing the facial skin.*

Just one session of wrinkle reduction treatment will last for approximately 4-6 months – meaning that the muscles won’t be creasing or folding your facial skin for the next 4-6 months.* If a series of wrinkle reduction treatment is done continuously every 4-6 months, the effect of each session may last longer, even beyond 6 months in a handful of people.*

As discussed above, think of your facial skin as a piece of cloth, if the cloth is ironed over and over again along the same line, a permanent line or pleat will form. This is a wrinkle. So if you stop ironing the piece of cloth along the same line, the pleat will get undone. The longer you stop ironing, the fainter the pleat on the cloth will be. The same principle applies for wrinkles.

Bunny lines are the creases around the nose bridge and bleeding lipstick lines abut the vermillion border of the lips, caused by the action of orbicularis ori muscle. Pebbly chins, also associated with mental creasing, are caused by the mentalis muscle on the chin. These muscles can also be relaxed by injection to reduce wrinkling.*

Possible side effects

These injections are skin deep, so side effects are usually of a superficial nature and highly transient. Needle marks and wheals only last for a few hours. Once they go away, there may not even be any sign of a recent treatment done. Occasionally, a bruise may occur over an injection site that disappears in a few days. You should abstain from lying down or massaging the areas of injections for 4 hours after a treatment session.

Facial Fillers & Facial Laser Treatments for Static Wrinkle Lines

This treatment mainly helps dynamic lines, i.e. wrinkling of the skin with facial muscle movement during smiling and frowning. It may soften permanent lines by preventing further creasing*, but has little or no effect in removing these permanently formed wrinkle lines.

What is hyperpigmentation? Why does it happen?

What is Acne / Pimples? What causes them?

Acne or pimple is an inflammatory condition of the pilosebaceous unit, i.e. the hair and sebum-producing gland, in the skin. These pilosebaceous units are found in great numbers in the face, chest and back of the body. Correspondingly, the common areas where acne occurs are also in these particular areas. Acne / Pimples forms in 4 stages:

(1) Overproduction of sebum (oil) in the pores;

(2) Formation of comedones (i.e blackheads and whiteheads) due to accumulation of sebum in the pores and shedded skin cells;

(3) Infection of nutrient rich sebum collection by bacteria Propionibacterium acnes; and

(4) Inflammation with formation of pus in pustules, nodules and cysts.

Acne / Pimple commonly starts at puberty, in the teenage years, but they can also start past the age of puberty in adulthood. Some suffer from acne way into their forties. The chest and back of the body is another place where acne / pimples can appear.

Acne / Pimple often causes people to feel defeated, depressed, devastated and embarrassed as they feel that their facial skin is un-presentable to the outside world. Acne Scars – Removal by Laser or INFINI Microneedling

What are some effective treatments for acne scars? Subcission

This requires manual separation of the skin from the deeper layers of the scars with a needle. The manual needling underneath the acne scars release scar tethering. It causes a little bleeding under the skin that allows the scar to then heal and level out, effecting remodelling of scars.* Bruising is common after this procedure.

Laser resurfacing – Fractional CO2

Ablative laser essentially shears off the top layer of the skin and heats it up so that it can re-heal into a smoother texture.* However, there is a higher risk with full ablative laser treatment. A less invasive and safer method: fractional resurfacing laser, which covers only 20% – 50% of the same treated area and therefore also has only 20% of the downtime and complications… and achieves good results too, even from the first session.* Usually around 5 to 10 sessions may be required.

Chemical peels including TCA peel

Essentially, this also aims to exfoliate the acne scars on the surface and/or excite new healing in the deeper layers, depending on the type of chemical peel used, thereby remodelling the acne scars. TCA is a medium depth peel that is particularly suitable to help smoothen acne scars, even resistant ones.*

Dermabrasion / microdermabrasion

Similar to chemical peels, except that it employs physical exfoliation. Dermabrasion may have excessive downtime so we recommend microdermabrasion instead, incorporated in our TY Microdermabrasion Facial for SGD 280 per session only.

Punch excision

At the very worst, stubborn acne scars may be removed surgically… but of course, this may lead to another scar and is a method reserved for the most unrepentant scars. We do not routinely recommend this treatment due to the possibility of further scarring after excision. (Currently not available in our clinic.)

Injection Medication

This is only suitable for raised or hypertrophic scars. It is effective in softening and flattening them, so that they are leveled with the skin.* Thereafter, laser treatment can help to improve them further.*

Acne Scar Filler

Filler can be used to fill out depressed acne scars and level them out with the skin.* It is only a temporary treatment and suitable for certain scars only. We use Restylane Vital Light as acne scar filler from SGD 400-680.

It is a myth that filler for acne scars lasts strictly 6 months only and is afterwards entirely resorbed. It is expected to last about 6 months in the body but recent few studies have also shown that it assists in the production of some collagen that gives acne scars more lasting improvement.*

Microneedling

The dermaroller consists of tiny needles that can be applied all over the face to release acne scar tissue and cause rehealing, somewhat like a milder form of subcission. For a more effective microneedling treatment, please read on…

Radiofrequency (RF) Microneedling by INFINI (Lutronic)

Infini

This treatment uses Fractional RF Microneedles, using a machine device that effects microneedling and delivers heat by radiofrequency, which can penetrate deeper than laser and improve stubborn resistant acne scarring.* It gives fractionated remodelling to the deep dermis and therefore also helps in skin tightening.* Usually 1-3 sessions are required.

It used to be that Radiofrequency in high energies cannot be delivered to the skin without causing a certain degree of epidermal damage or burn as it passes through the epidermis. However, this new technology INFINI makes use of microneedles to bypass the epidermis and reach the dermis directly so that the passage of energy is transmitted past the epidermal layer of the skin and gives selective heating of the dermis only. The needles that pass through the skin are insulated so that the epidermis layer is protected to minimise risks and side effects.

The result is that the deeper dermis, where the acne scars originate, is directly treated and effective remodelling of the acne scars is achieved.*

Fractional CO2 Laser skin resurfacing

This is a cost effective acne scar treatment commonly known to most as Fraxel. We only use FDA approved, HSA approved eCO2 laser system by Lutronic, which delivers just the right energy to give results with minimum downtime.*

Tiny beams of laser penetrate beneath the acne scars all over the skin so that acne scar tissue contracts and new collagen regenerates, therefore resurfacing the scarred area to restore a smooth complexion. This may be associated with some pain, which is alleviated by our strong topical numbing cream before the procedure.

Downtime is approximately 1 week, with some redness and swelling in the first 2 days. Scabbing follows for the next 3 days, followed by a quick recovery. Results are clearly visible with every treatment after a month or so, and even more with a prescribed course of treatments.* In fact, improvement may be seen up to 6 months post treatment.* Treatments should be spaced at around 4 weeks apart for maximum benefit.*

It is a myth to assume that acne scars cannot be treated. The Reality is that it is never too late to start improving the appearance of your acne scars for greatly improved confidence. Desirable results are usually seen after multiple repeated sessions of treatment.*

Possible Side Effects from Acne Scar Treatments

Sun protection and diligent post treatment skincare is very important to avoid post procedure hyperpigmentation. We adjust intensity of treatments to suit your skin type so as to minimise pigmentary changes that may be associated with aggressive treatment. Prophylactic abx may be recommended to pre-empt any possibility of infection. Downtime is usually from some bruising or redness or swelling, which takes a few days to a week to resolve. Results are visible from a month after treatment and acne scars may continue to improve up to 3 months after last treatment.*

For Stubborn Acne Scars

Do not think that there is only 1 treatment suitable for improving acne scars. With stubborn acne scars that resist standard laser treatment, these may improve with alternative treatments such as INFINI RF microneedling, TCA peels or subcission.* Most of the time, more than one treatment modality is necessary to achieve the best improvement of scars.* The best benefit may be achieved by combining a few of different treatments according to the acne scar types that you have, so keep an open mind when consulting with our doctors for the best regime for improving your acne scars.

Post Fractional CO2 Laser / INFINI RF Microneedling Treatment Care Instructions for You – updated

  1. You may wash your face from the first day with a gentle cleanser (recommended: Tiffiny Yang Skincare Gentle Cleanser) for the first week. Pat dry lightly – do not rub. Do not use exfoliatives or cleansing brushes.
  1. Apply antibiotic cream (recommended: Foban/Fobancort) for first 3 days.
  1. After washing of face, use a regenerating moisturizing cream (recommended: TDF Quick Recovery Cream) twice a day to assist in speedy healing.
  1. Apply cooling masks (recommended: Cryoderm Face Mask) to reduce redness and calm down your skin.
  1. 20 minutes of red LED light therapy may also be applied right after treatment to reduce redness and speed up healing of your skin.
  1. Sunblock (at least SPF 50) may be applied from Day 2 of treatment and is very important in preventing post treatment hyperpigmentation. Avoid direct sun exposure for the first week.
  1. Mineral cosmetics can be used from Day 3 of treatment to cover any signs of recent treatment.
  1. Retin A products / AHA e.g. glycolic peel/ hydroquinone may be used from day 5 of treatment to prevent post treatment hyperpigmentation, depending on the doctor’s recommendation for your skin.

What are Under Eye Circles?

Undereye circles are a result of a myriad of factors that add up to form the dark rings under the eyes.

First and foremost, there are many capillary vessels around the eyes that give the skin a dark purplish colour. Thereafter, with sluggish flow in blood circulation, hemosiderin deposits may increase and at the same time, pigmentation under the eye can increase. This happens when you are tired or sleepy and especially if you smoke. Exercise and regular eye massages may help, as well as eye creams containing Vitamin K.

Thirdly, thinning of the skin around the eyes exacerbates the problem, as the pigmentation underneath the skin and the color of underlying muscles is not well camouflaged, becoming more apparent. What are fillers?

How many types of fillers are there?

In a nutshell, fillers can be classified into temporary, semi-permanent and permanent fillers. Our favourite is the hyaluronic acid (HA), which is a temporary filler lasting between 9-12 months. It’s made of glycosaminoglycan polysaccharide and is not just found to be naturally occurring in the body as universal extracellular matrix– it’s also chemically identical across species and therefore, is very low on antigenicity, i.e. less likely to cause bad immune reactions. In other words, it’s very safe. There are many other brands in the market that are also similarly good in quality. Be sure to check with your doctor on this.*

There are also other type of fillers that are non-hyaluronic acid based, such as poly-L-lactic acid, calcium hydroxyapatite and poly-caprolactone. They essentially work by having an initial bulking effect plus an additional benefit of stimulating collagen synthesis wherever they are placed. This enhances their volume enhancing effect in the mid-term, so you may have to wait a little for the full effect. However, they may not necessarily last longer than current, newer type hyaluronic acid fillers. An alternative would be to choose a more viscous hyaluronic acid product, which your doctor can recommend to you.*

Semi-permanent fillers last the longest. Examples include polyacrylamide gel (i.e. plastic), and liquid silicone, which is the longest lasting of all fillers. These can last for years. However, the longer lasting the filler, the more likely it can cause complications such as lumps and scarring.*

Why did the facial filler seem to decrease so quickly in the first 2 days after injection?

Hyaluronic acid (HA) occurs naturally without cross linkages. In this form, it is easily broken down in the body in 1-2 days. The HA that remains are those that are still cross-linked, which prevents its breakdown, and this is the form that lasts. Therefore, your facial filler with HA can seem to decrease quickly in the first 2 days before it plateaus into a steady state. The rest decreases slowly. Therefore, the more cross-linked the HA, the longer it lasts.*

There is also a part of the filler procedure that causes a little swelling, such as the local anaesthetic and the inflammation of local trauma. This quickly subsides in a few days, leading to a reduction of up to 20-30% in the final result.

Should I always choose the brand of HA that lasts the longest to save cost?

The longer HA lasts, the more viscous it must be and therefore, the harder its feel may be or its chance of leaving residual lumps. Harder HA may not be suitable for all areas of facial correction, even though they last longer, because they may not be suitable for all areas or feel all that natural in others.

Why does my nose have a bluish tinge after filler injection with hyaluronic acid?

If it isn’t a bruise, it can be due to the Tyndall effect, which occurs after injection of hyaluronic acid is done too superficially so that it shows through the skin, making it take on a bluish tint that is not easily hidden by makeup. It doesn’t happen often, though, and can be avoided by placing the filler in the correct plane. Newer improved fillers seldom give the Tyndall effect these days.*

After a facial filler injection, can I go for massage or wash my face?

Temporary fillers are moldable in the first 4 hours or so and they settle in approximately 2 days. In the first 4 hours, you shouldn’t apply firm pressure on the area of the filler or massage too hard. Gentle washing of the face is permitted. For the first 2 days, it is advisable not to apply prolonged pressure on the area of the facial filler e.g. sleeping on that side of the face– there is a slight chance it may shift. Thereafter, once it has set, you can do whatever you want and it should not change in shape.*

Myths about dermal fillers– busted.

Myth: Fillers are permanent.

Truth: There are many different types of fillers. Some are permanent, some are semi-permanent and the rest are temporary fillers. Temporary fillers are mostly made of hyaluronic acid nowadays. It is a substance that is also naturally found in the body and is absolutely safe for use. It is gradually broken down by the body and absorbed, usually lasting from 6-12 months.*

Myth: Fillers are harmful for the body since they are absorbed by the body.

Truth: Absolutely not. Hyaluronic acid, which is used as temporary filler, is a natural substance found in the skin, joints and eyes of the body. It is gradually broken down by the body and not harmful to health.* In fact, some brands of fillers have obtained FDA’s approval for clinical use.

Myth: Surgery is required to place fillers into the body.

Truth: Not at all. Facial fillers for the nose, nasolabial folds, cheeks, chin, etc. are easily placed into the respective parts via a few simple injections.* There is no need to undergo the knife– hence the wonder of minimally invasive procedures. Even breast fillers only require 1 or 2 injection sites under the breasts. Minimal pain and minimal scarring, in just under 15min!*

Myth: You need to keep coming back for frequent top-ups once you do fillers.

Truth: Top-ups are required only because temporary fillers are naturally broken down by the body and absorbed, so its effect diminishes over time. They are not in the least bit addictive and you can choose not to top up again if you’d like. The filler will simply disappear and the part of the body will return to its original state as before filler was done.*

There can be incidences where the fillers last longer than expected in some people, especially after 1 or 2 top ups. This eliminates the need for frequent top ups over time.*

Truth: Fillers do not change the original state of the body part it is placed into. When it is absorbed, that part of the body simply returns to how it was supposed to be.* Of course, temporary fillers last up to a year or more and in that time, the body is still undergoing its own aging process.

Myth: Fillers are harmful to the body when used in the long term.

Truth: Hyaluronic acid is a natural substance found in the skin, eyes and joints. It is gradually broken down by the body’s natural metabolism and is not known to be harmful in the long term or short term. Newer studies have also showed that it may actually boost natural collagen in the skin for a more longer term effect, which can be considered desirable in many cases.*

Myth: Fillers change the shape of the body part permanently.

Truth: Not all fillers. Temporary fillers are made up of hyaluronic acid which are absorbed by the body over months and gradually, the body returns to its original state, as it was before.* Of course, as part of aging and change, the body does sometimes alter from its natural state but it is not usually affected by temporary fillers like these. Permanent fillers are not absorbed, though, and they can stay for a long long time. In that sense, they change the shape of the body part permanently.

Myth: Doing something to your face with fillers makes it look fake.

Truth: Amazingly, it is rather difficult to look fake with facial fillers.* They can give dramatic natural results and yet escape others’ eyes unnoticed.*

Myth: Fillers are very painful.

Truth: Some pain is to be expected but it is usually minimal and quickly forgotten after the procedure.* The pain is usually only of the injections and subsequently, a vague dull ache during filler injection. After the procedure, the dull ache may remain for a couple of days but it isn’t particularly noticeable and resolves quickly.* In other words, it definitely isn’t something you’ll remember for too long.
